Initial Client Interview

What to bring to your appointment:

The following items will help me get to know you so that I can analyze your case as quickly as possible, and leave more of your office conference available for me to give you specific information about your situation.  The items requested are not meant to overwhelm you and should not stop you from seeking legal advice.  If you have problems with any of the requested items, please discuss it with us.

  1. Client Intake Form.  Click here for .doc format | Click here for .pdf format Completing this form before your client interview will give us a much better idea of your financial situation and help expedite your debt recovery and end creditor harassment.

  2. Recent pay-stubs and last 2 years tax returns with W-2s.  >We need to determine your average income so bring a few stubs if pay fluctuates.

  3. All bills from creditors received in the last 60 days.  Please bring at least one “regular” bill (what you got while you were making your payments) from each original creditor. I also want any letters or bills that the creditor may have sent you asking for payment (they have different addresses), and any collection letters on each account.  (You don’t need to bring duplicates if the information looks the same, except for the amount owed).

  4. Lawsuits:  Copies of any lawsuits, judgments, garnishments, etc.
